Help with Chronic or Complex Illness
Aging Adult Services
Aging Adult Services Providing education, resources, and care coordination for adults age 65 and older residing in San Mateo or Santa Clara counties. Patients are offered services up to 90 days after they are discharged from Stanford Hospital or from skilled nursing facilities, to home. The Aging Adult Services team includes nurses, a gerontologist, a social worker and an occupational therapist.
Parenting At A Challenging Time
Parenting at a Challenging Time (PACT) A program designed to help communication and provide support and guidance to patients undergoing treatment for cancer. Stanford Senior Care Our team of geriatric specialists helps you better manage your changing health needs as you age. We offer comprehensive, interdisciplinary primary care and consultations to support patients and families coping with challenging conditions related to aging.
Supportive Care Programs
Supportive Care Programs (Neuroscience & Cancer) Wellness programs designed to provide patients and families with emotional, physical, and spiritual support through all stages of their medical care. We're here to help you strengthen your body, nurture hope and courage, and enrich your spirit. Our free support groups, classes, workshops, personal one-on-one consultations and services are open to all patients and families in the community regardless of where you receive care.

Back-Up Care
Back-Up Care for Adults and Children Use back-up care advantage when your regular caregiver is unavailable, your child's regular school is closed, your adult or elder relative needs care at home after surgery or illness.
Employee Assistance Program
Employee Assistance Program Talk to an EAP counselor for confidential support in your work and life. Each covered member can receive up to ten EAP sessions per issue, per year, at no charge.
